Specialist Domiciliary and Support for Children and Young People
At Triple Care Healthcare Services, we provide bespoke domiciliary services for children and young people aged 0–18 across Hampshire and surrounding areas. We understand the unique needs of each child and the importance of being supported in a familiar, safe, and nurturing environment.
Our homecare is centred on enabling children to:
- Remain safely at home with their families
- Receive tailored support that encourages growth, development, and independence
- Access care that respects their dignity, voice, and personal identity
Whether your child requires complex clinical care, assistance with daily living, emotional support, community integration or respite care, our trained carers are here to help.
We accept referrals from social workers, families, NHS teams, and individuals.

Free Home Assessment
Within 24 hours, we’ll arrange a no-obligation home visit with one of our Children and Young People Care Coordinators.
We’ll discuss:
- Your child’s daily needs and preferences
- Medical conditions or behavioural challenges
- Education, Health and Care Plan (EHCP) requirements
- Existing input from schools, therapists, or clinicians
Care Plan Creation
Our team will build a tailored, person-centred care plan in collaboration with you, your child (where appropriate), and any involved professionals.
We carry out:
- Comprehensive risk assessments
- Mobility and equipment reviews
- Medication and emergency protocol planning
- Matching of the most suitable carers
Once agreed, care begins — safely and smoothly. We provide consistent carers, regular updates, and ongoing reviews to adapt as your child’s needs evolve.

WHAT IS PERSON CENTRED CARE?
Person-centred care is the understanding that every person is unique, with their own set of values, beliefs, needs and preferences. A person’s needs or preferences are not static; a person changes and develops, and at Triple Care Healthcare Services at home, we understand this.
Our care planning and assessments are outcome-focused and will be shaped around each child or young person’s care requirement. We want to understand and support every child and young person in their development, both emotional and educational.
We have regular reviews, not only of our care plans but also the outcomes of our care delivery to ensure the children we are looking after are meeting or working towards their desired outcomes, and if they aren’t, we can reassess this and adapt.
