What is hospital discharge care?
The hospital discharge process can be a critical time for patients and their carers. After a period of illness they may be suffering from temporary or permanent impairment and may need assistance not previously required (including physical adaptations to their home and support services) in order to resume living at home. Discharge can thus be a point of life crisis and so needs to be handled with the full involvement of patients and professional carers. Every patient discharged from hospital is given a Discharge document with details of the patient and what they were treated for and all their past medical history. Three documents are produced; one goes to the GP whom we tend to work in the community with as well. All patients get discharged with a plan of action for the community to follow.

Who can benefit from hospital discharge?
Whether an individual has been in hospital for a long or short stay returning home can feel overwhelming. Being in a familiar place can be hugely beneficial in terms of an individual’s comfort and recovery. Triple Care can support an individual in regaining their independence or give the assistance with longer term support. Support can be given in an individual’s own home preventing unnecessary admission to hospital, nursing homes or residential care homes.

Triple Care provides care 24 hours a day, 7 days a week hospital discharge service in your own home. Our carers are well trained to meet an individuals need. We work with the multi-disciplinary team in the hospital to create a smooth transition. Having previously worked in hospital settings our qualified nurses understand the process and lead in the assessment so that individuals can safely return home. We will contact the hospital ward to receive a handover and support in getting everything in place for an individual’s return home.
In some cases individuals return home with physiotherapy or occupational health pathway. Triple Care works in conjunction with these services to build a care plan specific to the needs of the individual with the right equipment in place. Triple Care will work will community physiotherapists, occupation health professionals, dieticians and community nursing teams to be guided by their specialist input.
Our qualified nurses are able to assess if an individual is safe to be home. Upon discharge of our clients a member of our nursing team assess the medication list and can liaise with the discharge team on this.
