We help people live as independently as possible in shared or individual accommodation while receiving person-centred care tailored to their needs. Our homes are welcoming, community-based, and staffed by professionals who truly care.

We’ll listen to your situation, gather key information, and let you know what placements or homes are currently available.
Referral and Initial Assessment
We’ll request or accept a referral pack which may include:
- Care assessment or support plan
- Risk assessment or behavioural support plan (if applicable)
- Health history and medication list
- EHCP (if under 25), discharge summary, or transition plan
Once received, we will:
- Arrange an initial assessment with the person and/or their advocate
- Conduct a face-to-face meeting or video call (if needed)
- Visit the current environment (e.g., home, hospital, residential care) to ensure continuity
Matching and Property Viewings
We carefully match the individual’s needs, personality, and preferences with a suitable supported living property.
We’ll arrange:
- Home visits or virtual tours of available placements
- A chance to meet potential staff and housemates (if shared)
- Discussion of preferred routines, privacy needs, and compatibility
If needed, we can support overnight stays, trial visits, or phased move-ins to reduce anxiety and support smooth transitions.
Transition Planning and Move-In
Once a placement is agreed:
- We create a detailed transition plan tailored to the person’s support needs
- Move-in logistics are handled collaboratively with family, carers, social workers, and any other key professionals
- A Support Agreement and Tenancy Agreement are issued and explained clearly
- Our team ensures that medication, equipment, appointments, and specialist input are arranged
Personalised Support Begins
From day one in the new home:
- A named Key Worker will support the individual closely
- Staff deliver support in line with a fully personalised care plan
- We build routines, structure, and daily stability
- Any emotional, behavioural, or transitional concerns are supported with compassion
Who Can Make A Referral?
We welcome referrals from:
- Local authority adult social care teams
- NHS discharge teams or mental health services
- Parents or family advocates
- Supported decision-makers, IMCAs, or legal guardians
- Individuals self-referring or seeking direct payment/self-funded support

What Is Supported Living?
Supported Living at Triple Care is a housing-with-care model — you or your loved one live in a safe, shared, or self-contained home while receiving tailored care and support from our trained team.
You’re supported to:
- Make your own choices
- Live independently with help where needed
- Manage your health, routines, and relationships
- Connect to your local community
- Set and work toward personal life goals
You maintain your tenancy and housing rights while having access to flexible care that fits around you.
